***The immunization office will not accept any medical/vaccine documents that are signed off or stamped by family members that are licensed to prescribe***

• Proof of two (2) MMR vaccine doses received at least/minimum 28 days apart, must have been received AFTER your first birthday. Vaccinations MUST have been received in 1971 or later OR  proof of immunity by way of a blood test result (Rubella/Rubeola titer). Must upload positive lab documentation through your SHS portal.
• The official State of Florida Immunization Form (680, blue form) issued by local "Health Departments" & physician offices (must include stamp)
• Childhood immunization records: (the little booklets, not family baby books/album pages)
• High Schools: Most public schools require the MMRs K-12  (you will need to speak with that school to request them)
• High school transcripts: May hold vaccine dates (if sent to Admissions you will need to upload the vaccine documents page to yout SHS portal as we do not share information) 
• Military documents: must have full dates MM/DD/YY not a discharge date (DD214 not valid)    "Infinite Military Titer" is not acceptable. We must have actual lab documentation.
• FGCU FORM: If you can't locate documents from the sources above, the "FGCU Immunization History Form/Parent Waiver" can be completed & uploaded to your portal. Make sure it is stamped and signed by authorized personal (NOT AN FGCU MEMBER) (see "Printable Immunization Forms")
• FLShots:    ***If you've received vaccines in Flordia or transferred vaccine documents to an establishment in Florida, they MAY already be uploaded to your SHS Portal by the "FLShots Database System". You can check under the My Profile section to see if you are compliant with all immunization requirements. 
• Exemption: Medical/Pregnancy/Illness/Age, please see "Valid Immunization Exemptions" below.

You will be exempt from the State of Florida University System immunization requirements if you meet any one of the following:

1. If you are a Distance Learner who is living or working OUTSIDE the United States and are able to provide proof please contact this office at 239-590-1254.
2. Birthdate Exempt if born prior to December 31, 1956 you will need to call the Immunization Department at 239-590-1254.
3. Special Circumstance(s) Exemptions:
   
   1. Pregnancy/Fertility Treatments: On a dated office/physicians letterhead please specify PT name, DOB & expected date of delivery if pregnant. Please provide brief            explanation of event & include physician's signature and contact information (medical provider cannot be that of a family member).
   2. Breast-Feeding/Lactation Considerations: Measles, Mumps & Rubella (MMR): Excretion in breast milk unknown. Please discuss receiving the MMR vaccine with your pediatrician while breast-Feeding.  On dated office/physician鈥檚 letterhead please specify PT name, DOB, you are breast feeding and that the physician does or does not support receiving the MMR vaccine. Include physician's signature and contact information (medical provider cannot be that of a family member).
   3. Illness/Disease: A written, current dated statement on office/physician letterhead signed by a physician and including his/her contact information.  Please specify the students' name, DOB, date seen and that the MMR vaccines cannot be given as student is immunocompromised. (medical provider cannot be that of a family member).
   4. Allergy: A written, dated statement on office/physician letterhead signed by a physician including his/her contact information which specifies the date seen, type of reaction and a brief explanation of the event (medical provider cannot be that of a family member).

• FGCU Student Health Services (must have a University ID Number to be seen here) Please View the  "cost" tab for pricing.
• Walk-in clinics.
• Your private physician office.
• Health Department.
• VA (if active military or veteran).
• CVS/Walgreens/Publix/Minute Clinics.

If you DO NOT have vaccination records from another institution the "Immunization Form" below should be taken to a doctor, completed, signed, stamped & uploaded in your portal.  As the parent, if using this form to opt out only then NO office stamp/signature is required. Must upload to portal.

  • For individuals with health insurance we recommend considering alternative locations such as CVS, Walgreens, or Publix Pharmacy.

• TDAP - $43.00 Tetanus, Diphtheria & Acellular Pertussis (According to CDC guidelines as an adult, you should have one "pertussis"  tetanus booster, TDAP)
• Color Vision Screening- No Charge
